Case Management & Service Coordination

Service Coordination is an individually centered service focusing on consumer strengths, needs, and abilities. Service Coordination works with individuals to assess needs, set goals, refer to various services, and advocate for consumer needs and rights. The individual must have a need for the service. Examples of services funded include vocational services, transportation, representative payee services, and ongoing general assistance. Polk County funds both this program and the services needed by consumers.

Eligibility Criteria

  • At least eighteen years old
  • Have a documented primary mental health diagnosis
  • Generally requires only one service and minimal assistance from the Service Coordinator in order to maintain that service
  • The majority of consumers in this program do not have any financial or insurance benefits
  • Individuals who are in the process of applying for disability benefits may be eligible to receive monthly financial assistance until the determination process is completed

How to Make a Referral

When calling to make a referral, please have the following information available:

  • Has the client been hospitalized in the past for mental health reasons?
  • What services is the client in need of at this time?
