Integrated Services Project

Integrated Services Project (ISP) provides nontraditional and creative services to adults with chronic mental illness. Services are voluntary and focus on helping members meet self-defined goals and fostering independence and self-sufficiency. ISP staff coordinates or purchase services and provide direct service to members. Staff assists in enrolling members in traditional services and teaches members how to access community resources and develop community supports.

ISP emphasizes employment and education. An Employment Specialist provides services related to employment and provides educational opportunities to meet needs. The ISP oversees the Opportunity Center which is a space that allows members to socialize, watch TV, do laundry and spend time on the internet. The purpose of the Opportunity Center is to assist members in staying connected socially and to orient members to new entertainment and activity options. The staff Activities Coordinator facilitates the operation of the Opportunity Center.

Eligibility Criteria


  • Diagnosis of a chronic mental illness
  • Legal settlement and current resident of Polk County
  • Has received ongoing supportive services for at least six months through Service Coordination
  • Requires ongoing services in at least two or more of the following areas:
    • Housing
    • Transportation
    • Employment
    • Skill Development/Education
    • Financial Support
    • Mental Health Services
    • Recreation
    • Basic Needs
    • Coordination and Development of Natural Support Systems
    • Rehabilitation
    • Crisis Intervention

Funding

Funding is provided by Polk County Health Services at a flat monthly rate per member. There is no charge to the client.

How to Make a Referral

Referrals are received as assignments from Polk County Health Services; however the intake process begins with the Golden Circle/ISP intake coordinator. The intake coordinator can be reached at 515.241.0982. The intake coordinator will assess eligibility. A qualified individual will then be placed on the Polk County waiting list and assigned to a Service Coordinator for no less than six months for ongoing supportive services and evaluation of services needed. When an individual comes off of the waiting list, Polk County Health Services assigns the approved individual to one of three Polk County Integrated Services Projects.
