Program for Assertive Community Treatment

PACT is a team of professionally trained individuals working to provide comprehensive home and community based mental health and vocational service to persons with chronic and persistent mental illness. Services are individualized to meet the unique needs of each consumer and address all areas related to successful community living. The PACT team provides both case management services and direct, hands-on support services.

PACT Goals

  • Lessen or eliminate the debilitating symptoms of mental illness and minimize or prevent recurrent acute episodes of the illness
  • Facilitate successful community living by providing intensive community supports as needed
  • Assist clients in meeting basic needs and enhance quality of life
  • Broaden the support system available to the consumer, including professionals, family members, and significant others
  • Assist with and facilitate vocational and social roles in the community

PACT Services

  • Extended hours: 12 hours a day, 5 days a week
  • Crisis intervention: 24 hours a day, 7 days a week
  • Psychiatric treatment
  • Symptom education and management
  • Supportive counseling and individual therapy
  • Skills teaching
  • Substance abuse (Dual Disorder) treatment
  • Vocational Support
  • Medication management and observation

Eligibility Criteria

PACT serves Polk and Warren County residents who have difficulty utilizing traditional services and have high service needs such as:

  • High use of psychiatric hospitals or emergency services
  • Co-existing substance abuse disorder
  • High risk or recent history of criminal justice involvement
  • Difficulty meeting basic survival needs, such as housing
  • Difficulty maintaining consistent employment

PACT services are funded through Title 19 and are contracted through MBC of Iowa with priority given to persons living with Schizophrenia, Bi-Polar Disorder, and other Psychotic disorders.

How to Make a Referral

PACT accepts self-referrals or referrals may be made by a physician, psychiatrist, therapist, case manager, family member, or another organization. Referrals are accepted at 515.282.9972.
