Clinical Outpatient Program

Clinical outpatient services are provided at 1301 Center Street in Des Moines and at 301 N. Buxton in Indianola. Clinical social workers and mental health counselors make up the well-qualified therapy team offering individual, group, couple and family therapy. Services include assessments and counseling for a wide range of issues/diagnoses including, but not limited to, depression, anxiety, schizophrenia, bipolar disorder, anger management and co-occurring mental health and substance abuse disorders.

In addition to therapy, the clinical program has an experienced team of psychiatrists, nurse practitioners, and mental health nurses that offer psychiatric assessments and medication management.

The clinic is open five days a week from 8:00 until 4:30. Evening hours are available on Tuesdays until 8:00 pm. Psychiatric services are provided five days a week, also including some evening hours on Tuesday.

Eligibility Criteria

The clinical outpatient program primarily serves persons age 17 and older residing in Polk and Warren counties.

Payment for Services

Eyerly-Ball accepts payment from all major insurances, Title XIX and Medicare. A person legally settled in an Iowa county who is without insurance and unable to afford the full service fee can apply for a sliding fee. The service fee is based on household income and family size. At the time of initial call, the intake coordinator will gather necessary information and verify coverage and payment arrangements prior to the first clinical appointment.

To schedule your first appointment the following information will need to be provided: name, address, phone number, Social Security number, birth date, referral source, services requested, income, insurance coverage, and length of time at present address if county funded.
