What is Eyerly-Ball Community Mental Health Services?

Eyerly-Ball provides high quality recovery focused adult outpatient mental health services. This includes individual, couple and group counseling as well as psychiatric assessments and medication management. Services are provided by an experienced team of psychiatrists, therapists, a nurse practitioner and a mental health nurse.

Monday – Friday 8:00 am to 4:30 pm
Tuesdays – 11:30 am to 8:00 pm

Clinical Services

Eyerly-Ball Community Health Services offers a full range of professional mental health services:

Eligibility, Fees and Payments

Services are provided to persons 17 years and older.

All major insurances are accepted, including Medicare and Medicaid or private insurance. We will complete and file the proper forms for you at no cost.

Co-payment and deductibles for services must be paid at the time of service unless prior arrangements have been made.

Emergency Care

During regular business hours, staff responds to emergencies as they arise. To speak to your therapist or other clinical staff, please contact the office at 515.243.5181.
For after hours emergencies, or on weekends and Holidays, please call 515.243.5181 and follow the recorded instructions.
